Position Overview:

As a Remote Primary Nurse, you will deliver compassionate, comprehensive, and evidence-based care to patients managing chronic conditions. You’ll collaborate with a multidisciplinary team, utilize cutting-edge telehealth technology, and empower patients to achieve better health outcomes.

Key Responsibilities:

  • Serve as the primary point of contact for chronic care patients, offering support, education, and personalized care plans.
  • Conduct telephonic or video assessments to evaluate patient needs, progress, and adherence to treatment plans.
  • Coordinate care with healthcare providers, specialists, and community resources.
  • Monitor and adjust care plans based on patient outcomes, ensuring optimal management of chronic conditions.
  • Provide education on disease management, medication adherence, and lifestyle modifications.
  • Document all patient interactions and maintain compliance with clinical protocols and HIPAA regulations.

Preferred Skills:

  • Experience in telehealth or remote nursing roles.
  • Certification in chronic care or case management (e.g., CCM, CMCN).
  • Bilingual skills are a plus.

Qualifications
  • High school diploma or equivalent (Bachelor’s degree preferred).
  • Previous experience in customer service, preferably in the travel or airline industry.
  • Strong verbal and written communication skills.
  • Proficiency in using computers, including familiarity with CRM systems and Microsoft Office.
  • Ability to multitask and work in a fast-paced environment.
  • Strong problem-solving skills and a customer-first mindset.
  • Reliable internet connection and a quiet workspace free from distractions.
